I am a certified, full-time police officer for the City of Lawton, Oklahoma, Request to be admitted to the membership of the International Union of Police Association, Local 24, AFL-CIO. I agree to abide by the International Constitution and the Local By-Laws, and understand that any violation will subject me to the sanctions of the Local, pursuant to the By-Laws.

I further authorize the deduction from my bi-weekly pay from the City of Lawton, Union Dues in the amount of $50.00 dollars. I will be required to notify the Payroll Clerk, with the City of Lawton, in writing to begin withdrawing Union Dues. My membership will remain in good standing as long as my dues are withheld, and/or paid to the Local.

In the event I no longer wish to be a member of the Local, I will notify the Local in Writing, and will submit a written request to the Payroll Clerk to have the deduction of the Union Dues stopped. I understand that it is my sole responsibility to have the deduction stopped in the event of my resignation from the Local.
